I understand that participation in futsal and soccer activities — including practices, tryouts, games, and related events organized by Joga Labs — involves inherent risks, including but not limited to falls, collisions, contact with other participants or equipment, and other risks that cannot be eliminated even with proper supervision. On behalf of myself and the player named above, I voluntarily accept and assume these risks. To the fullest extent permitted by Texas law, I hereby release, waive, and discharge Joga Labs, its owners, coaches, staff, and volunteers from any and all claims, demands, or causes of action arising out of or related to any loss, damage, or injury sustained by the player while participating in Joga Labs activities, except where caused by gross negligence or willful misconduct. I agree that this release is binding on me, the player, and our heirs and legal representatives.

In the event of an injury or medical emergency, if I cannot be reached at the numbers provided, I authorize Joga Labs staff to secure emergency medical treatment for the player, including transportation by emergency services. I understand that I am responsible for any resulting medical expenses.

Joga Labs may photograph and record players during club activities and use such images and video — without compensation — for club purposes including the Joga Labs website, social media, and YouTube channel. Players are never identified by full name without separate consent.
